Longevity Vital Signs

When most people think about longevity, they focus on living longer. But the real goal isn’t simply adding years to life—it’s adding life to those years.

This is where the concept of health span comes in. Health span refers to the number of years we remain healthy, active, independent, and capable of doing the things we love. While lifespan measures how long we live, health span measures how well we live.

So how do we know if we’re building a body that supports a long, healthy life?

Researchers and clinicians increasingly rely on a few key performance markers that provide valuable insight into long-term health and functional capacity. Among the most important are muscle mass, grip strength, and cardiorespiratory fitness.

Muscle Mass: The Foundation of Healthy Aging

One of the strongest predictors of healthy aging is maintaining adequate muscle mass throughout life.

As we age, we naturally lose muscle tissue—a process known as sarcopenia. This decline can affect strength, balance, mobility, metabolism, and overall independence. Preserving muscle mass helps us stay capable of performing everyday tasks, from carrying groceries to climbing stairs and getting up from the floor.

We look at body composition with DEXA, and we look at a few different ways to look at your quality of movement, your mobility, your stability, your ability to create different shapes and patterns in a way that’s conducive to those functional tasks that we would expect to be able to do, when, as we age.

One of the best ways to assess muscle mass is through a DEXA scan (Dual-Energy X-ray Absorptiometry). Considered the gold standard for body composition analysis, a DEXA scan provides a detailed look inside the body, measuring:

  • Lean muscle mass
  • Body fat percentage
  • Visceral fat
  • Bone mineral density

This information offers a much clearer picture of health than body weight alone.

For both men and women, maintaining healthy muscle mass should be a priority. While body fat percentages can vary among healthy individuals, muscle tissue serves as a critical reserve that supports metabolic health, physical performance, and resilience as we age.

 

Don’t Forget Bone Health

An added benefit of DEXA scanning is its ability to measure bone mineral density. Strong bones and strong muscles work together to reduce injury risk, improve mobility, and maintain independence later in life.

When evaluating longevity, it’s not enough to look at what we weigh—we need to understand what our bodies are made of.

Grip Strength: A Simple Test with Powerful Insights

Among the many longevity markers gaining attention, grip strength stands out for its simplicity and predictive power.

At first glance, grip strength may seem like an odd metric. Why would the strength of your hand tell us anything meaningful about your overall health?

The answer is that grip strength acts as a proxy for the health of your entire neuromuscular system.

When measured with a dynamometer, grip strength doesn’t just assess the muscles in your hand and forearm. It reflects how effectively your brain, nervous system, and muscles work together to generate force.

Think of it this way:

  • Muscle tissue is the hardware.
  • Your nervous system is the software.

Grip strength gives us insight into both.

It’s not only about how much force you can produce, but also how efficiently your body can recruit muscle fibers and transmit signals from your brain to your muscles.

Researchers have repeatedly found that lower grip strength is associated with increased risks of disability, frailty, hospitalization, and even mortality. Conversely, stronger grip strength is often linked to better overall health outcomes and functional capacity.

That doesn’t mean everyone should spend hours training their forearms. Rather, grip strength serves as a quick snapshot of total-body strength and neuromuscular health.

Strength Is More Than Muscle Size

Muscle mass is important, but muscle size alone doesn’t tell the whole story.

What truly matters is your ability to produce force.

In exercise science, we often evaluate how much power a person can generate. Power is the combination of strength and speed—the ability to produce force quickly and effectively.

A useful analogy is to think of the human body like a high-performance engine.

When people compare vehicles, they often discuss horsepower and engine size because those factors determine performance. Similarly, our muscles generate power that allows us to move, react, and perform daily tasks.

Grip strength provides a practical glimpse into that power-producing capability. It tells us not only how much muscle we have, but also how effectively that muscle can be used.

The Other Side of the Longevity Equation: Aerobic Capacity

While strength and muscle mass are essential, they represent only one side of the longevity equation.

The other side is aerobic capacity—our body’s ability to produce energy efficiently during sustained activity.

This is where measurements such as VO₂ max come into play.

VO₂ max measures the maximum amount of oxygen your body can utilize during exercise and is widely considered one of the strongest predictors of cardiovascular fitness and longevity.

If strength determines your ability to generate force, aerobic fitness determines your ability to sustain effort.

Together, they create the foundation for lifelong performance:

  • Muscle mass supports resilience and metabolic health.
  • Strength supports function and independence.
  • Aerobic capacity supports endurance and cardiovascular health.

All three contribute to a longer, healthier health span.

When it comes to longevity, the goal isn’t simply to live longer—it’s to remain strong, capable, and independent for as many years as possible.

Two of the most valuable vital signs of healthy aging are surprisingly straightforward:

  • Muscle mass, which can be assessed through tools like DEXA scans.
  • Grip strength, which serves as a powerful indicator of overall neuromuscular health and functional strength.

Combined with strong cardiovascular fitness, these markers provide a meaningful picture of how well your body is aging.

The question isn’t just, “How long will I live?”

A better question is:

Will I still be strong enough to enjoy the life I’m living?
